Change Futures is committed to respecting your privacy through the effective management of personal information in accordance with the provisions of the Commonwealth Privacy Act (1988), National Privacy Principles, and where applicable, the Information Privacy Act 2009 (Queensland) and the Health Records and Information Privacy Act 2002 (New South Wales).

You and/or your legal guardian/nominated advocate (if applicable) will receive an explanation about privacy, confidentiality and the exceptional situations where your confidentiality may not be protected. This will occur at the point of intake and during your time with our service as required. You will also have an opportunity to discuss this with your practitioner.

A file will be created for information that is relevant to providing support services to you. The personal information we collect depends on the type of services you are accessing and may include (but is not limited to):

  • Name and contact details
  • Medical information
  • Information relating to support needs
  • Information relating to goals and aspirations
  • Representative, advocate, case manager or carer details
  • Guardian or administrator details
  • Cultural background and / or preferred language
  • Other circumstances which may impact upon your care needs
  • Emergency contact detail
